1. Infringement Notification
If you are a copyright owner or authorized agent, prepare a notice using the Report Content worksheet. An effective notice should include:

- Identification of the material that is claimed to be infringing, including the exact URL(s) where it is located;
- Your contact details, including name, mailing address, telephone number, and email address;

- A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner.
2. Counter-Notification & Repeat Infringers
If content was removed by mistake, a counter-notice generally identifies the removed URL and includes the required jurisdiction and signature statements. The worksheet does not file it; a completed notice must be delivered independently to the appropriate recipient.
